You can call 1-800-772-1213 to find the closest Social Security Office to your home and schedule an appointment to file the Request for Hearing. You also complete an updated form about your medical condition since your applied. You can bring a family member or friend to help you fill out the forms. Don’t forget to bring the names and addresses of medical providers you have seen since you filed the Request For Hearing together with a list of any new medications you are taking. Bring copies of any new medical records.

2. Apply Online

You can apply for benefits at www.ssa.gov by clicking on “appeal a decision” and click on “Request for Hearing.” You can stop and start the Request for Hearing. Contact Ms. Cavey if you have problems.
